In preparation for your first appointment, please print and complete the below client intake form to bring with you to your upcoming Liminal Interlude sound healing and crystal gridding balance. Please note that you do not need to complete this form if you have previously completed a Kinesiology or Equilibrium client intake form as your details will still be on file.

This form will also be available for completion on the day in case required, however please arrive 10 minutes early to complete this prior to your appointment.
